Does anyone know if this grate can be lifted? It’s very old and there’s lots of gunk in there including a comb that I’d like the remove to prevent it blocking pipes
 
Yes it was made to be able to be unscrewed - by turning anti clockwise.

Chances are the grate will snap when you try to do it.
Spray some lubricant spray around the groove of the grate and see how you go.
If you snap the grate you are going to find it difficult to find something that fits in its place

Make a special tool to unscrew it. Stout 2inch x 3 inch say 3 feet long wood, drill and screw some big fat 3 inch screws into the corresponding places to insert the screws or bolts
into the furthest apart slots, spray with wd40 hours before, insert into grating and start off gently then give the end of the tool a sharp wack..if stuck.let us know how you get on

Making a tool to fit the smallest furthest apart slots is very good advice, it's much less likely to snap the grid there. I would use plus gas releasing fluid or similar rather than wd40, pour unto the gap and leave at least overnight then repeat. If still stuck, you could consider the application of heat to the grid which might free it up. All this trouble is much easier than replacing a broken grid!
