hi guys just been called to fix a leak which i have fixed twice already and has apparently been fixed several times in past. It is a hot water tee originally a john guest speed fit tee linking 2 copper 1 plastic. The tee itself had a gouge in on 1st visit. replaced with a conex tee. 4 days later the plastic pipe was blown out the tee. Figured must have over tightened so trimmed back the pipe and refitted. Lasted 2.5 weeks and just been callf again.been told water was hot depite hot tap not being on although could just of heated quickly. Pipe unfr boiler and boiler has a drip on prv after an expansion vessel fault. Any ideas?

Correct insert used? Copper olive?

dont have any on van going to have to do a temp fix and go back
