How easy would it be to change this monstrosity to exposed mixer shower?

He said it's not electric. Have never installed anything like this so don't know what would be behind it.

In short, would there almost certainly need to be a retile (partial or otherwise) or not?

There'll just be a hot and cold feed into it behind. It's just a tower shower.

Looks like its come from the showers at JJB gym.

usually a couple of screws in the bottom pull out and then lift up its normally just on two flexis coming out of the wall not normall fitted before the tiles so there should be tiles behind it
