Looks like it mains in the top, hot out of the side close to the top of the cylinder part.

How is it heated can't see cable or immersion?
 
Wheres the vent pipe? Usually you can see the pipe going up? Can you see anything on the inside of the cold part?

I've never seen one like this but that's only me.
 
It looks like a combination cylinder, but the draw off is very low.the vent will be internal.
