I use a thick foam kneeler but have started to notice worrying signs from my knees anyway. Wondering if a gel knee pad might be better? Or even combined the two.

I don't wear work trousers with pockets for pads so guess would need to be strapped on type. Sounds annoying but needs must....
 
I use a pad like you!
I strayed from the path and bought various things.
Tried gel knee pads. Straps cut blood off when kneeling, they fall down when standing, they move knee caps about in a torturous manner when crawling.
My experience led me back to pads like you.
Other gel knee pads may be OK. I can't say they'll all be the same.
 
I have trousers with pockets in for pads. I also have an old cushion folded in half when I'm on the floor for any length of time.

I tried the strap on knee pads but found the straps dug into my legs.
 
Redbacks but you need the trousers pockets
 
Foam inserts in trousers ok for when moving around a lot.

I also didn’t get on with my strap on gel pads, too loose or too tight couldn’t get it right.

When in one place for a while I use an old sofa cushion this has added advantage that as you sink in it keeps you warm too - great today in an unheated warehouse at about 3 degrees.
