No real experience with soil and vent, not sure what to do here

Changed an AAV once. But apart from that I've never worked with it or sorted probs with it.

So not sure what needs to be done here.

Lady is deaf and communication is difficult so quesitons were kept to a minimum.

Bath doesn't drain and when basin used it bubbles up into bath. Same with kitchen sink. Worked previously.

The picture shows what's on otherside of bathroom wall where bath, basin and kitchen sink wastes terminate.

When cap is unscrewed everything drains fine.

What's the fix?

I don't think that's an AAV - if you look carefully you can see threads. It's a cap. I'd stick a Durgo on it...

It has clearly been in for a long time, so ask yourself why has it just started doing this ? Why would adding an expensive Durgo now make a difference ?
Me, I would be checking the drainage system below ground.

Surely fitting a Durgo is just masking the root of the real problem, possibly a blockage between there and the existing vent.

Classic sign of a blockage downstream. When you have a blockage further down the line, positive pressure build up inside the stack, meaning appliances drain very slowly, unscrewing that cap (which is most likely an air admittance valve) relieves this positive pressure allowing the water to drain away.

You'll probably find that if you empty a bath with the cap removed the stack will eventually fill with water, this is the perfect opportunity to get a rod with a plunger attachment and use the body of water to force the obstruction away. If not you'll have to attack the blockage with something more substantial.

cut the 4" just under the top tee , and fit new one then extend 4" to a high level and do take few attempts to flush it and if it does then fit new durgo (must be the internal type) and do test again before leaving job ! also there must be ventilation to the boxing in order the durgo to work

it could well be a problem further down the system, i would fit a dergo either way, make sure its removable and vented. I tend to find if its being plasterd or boxed in then its best to terminate in in the loft or with a 9x9 vent infront of it for access at high level.

As Chris said there is a partial blockage further down the line which need dealt with. An 1½" pipe discharging into a 4" will never fill it stopping the air flow unless something is preventing it running away.
Fix that problem first.

Btw that is a screwed access cap. Cut it below the threads and glue a loose collar on then extend the 4" above the spillover level before fitting a durgo but fix the drain first.

i did clock that and i'll mention it. even if the risk is tiny, warranty voiding 'n' all....
