NVQ Level 2 Portfolio (on site work)

I am currently doing my level 2 Plumbing NVQ and have been told to get photo evidence of the work im doing and put it in a portfolio and include write ups with the pictures attatched.

I was wondering if anyone could tell me where i could find an example of a completed portfolio or at least a write up about a certain job so i can see how to word it and what sort of things i need to include on my write up.
