Called to install a cooker for someone yesterday. Expecting just the usual fit the hose and do the normal safety checks. HoweverIwas very alarmed at the explosive way one of the burners lit. Further checking showed that in the LPG conversion the burner injectors had all been swapped diagnonally - so we had two over-gassed and two under-gassed. Inclination is to RIDDOR this as it is sloppy manufacturing. However I am not sure that it counts as either a dangerous fitting or an inflammable gas incident! What is the concensus?
 
If only we could report manufacturers.

I wen't to install a B/F Gazco E-Box a few months ago only to find they'd not sealed the box fully at the top and it was allowing the fumes to vent straight into the room.

Also heard of a boiler manufacturer putting the wrong burners in appliances.

This is why we do our checks, to ensurer everything is ok and is as should be.
