If its an old style like you say with a handle, then diaphragm has gone. Just get whole thing replaced

This is my toilet.

Like i say, i have to flush it 3 or 4 times rapidly and then flushes.

I thought it may have been the fact that it was loose, but after i tightened it, it stayed the same.

Is it the defo the diaphragm?

If so what type do i need?

to change, turn off water to cistern if iso is there, if not isolate at main stopcock. pull flush to drain as much water from cistern as possible, then undo the two wing nuts holding your cistern onto the pan, and undo the feed pipe thats connected to the fill valve.

Lift off cistern and pour out remaining water into pan. undo the nut holding the flush valve in place. remove old one and fit new one. you will also need one of these:
