When doing any gas work for customers what paperwork do you need to complete?

I understand you need to register new installs, what about smaller jobs such as servicing and repairs what forms need filling in if any?
 
Its not compulsory, but good practice to fill out a service/home owner type sheet for any job you do.
The bigger companies will do that as a matter of course.
