i found one like that when I worked at the council.

No one had ever changed the earth pins and the insulation had fell onto the burner causing the flame to burn through the heat exchanger.

I found it on a service, the boiler had been working fine and the tenant hadn't reported a breakdown or anything.

God knows how long it had been like that for.

Had two early 80s oil-fired warm air units in a factory unit 'up north'. When I checked no-one knew if they had ever been serviced since the place was built. Both flues missing caps and one unit heat exchanger rusted through so fumes blowing into the factory, though as they had vehicles in the place nobody noticed. Got other unit working after a fashion but condemned the first. Told them to replace both which they did ...... about 5 years later!
